Care Management Associates, a division of Connecticut Community Care, Inc., helps elders and adults with disabilities find the resources needed to live as independently as possible. We provide comprehensive geriatric assessments, case management, and quality home care services. Our care managers are nurses and social workers with years of experience in care management. We are members of the National Association of Professional Care Managers. In 2007, we were chosen by Connecticut’s attorneys and law firms as “The Best” in Home Care through a survey done by the Connecticut Law Tribune.
